- The youth enrollment form is a form that needs to be filled out each year. THe enrollment form is used for contact information as well as information for 4-H projects. For more information about filling out the enrollment form please see the 4-H Enrollment Guidelines.
- Due: For past members the enrollment form is due in November, while new members can join at any time throughout the year.
- Member Expectation Statement (.pdf)
- The Expectation Statement for Youth is a form that needs to be filled out each year. The Expectation Statement for Youth is used to inform youth about what is expected of them when participating in 4-H.
- Due: Due when Youth Enrollment Form is turned in
- Leader Enrollment Form (.pdf)
- The Leader Enrollment Form is to be filled out each year. The enrollment form is used for contact information, to inform what types of leaders clubbs have, along with the expectations for leaders.
- Due: December 1st for returning leaders and anytime for new leaders
- Leader Volunteer Application (.pdf)
- The Leader Volunteer Application is a form that needs to be filled out by new volunteers. This application enables the 4-H Youth Development program to provide a safe environment for the youth it serves.

Awards
- Amanda Bresina Horse Camp Award (.pdf)
- The Amanda Bresina Horse Camp Award is given to a Taylor County 4-H Horse Project Member to help with the cost of attending a horse camp of choice.
- Due: August
- Bernice Brandner Memorial Leadership Award (.pdf)
- The Bernice Brandner Memorial Leadership Award is to recognize leaders that demonstrate the same values and dedication to 4-H as Bernice Brandner did.
- Due: September
- Robbie Sanders Memorial Rabbit Project Application (.pdf) | Robbie Sanders Memorial Rabbit Project Information (.pdf)

- Creative Club and Outstanding Leader Awards
- It’s very simple….if you want to nominate a club or leader for either or these awards, just send in a one page or less description of why your nominated leader or club shoud be selected.

- Key Award (.pdf)

The Key Award is for special recognition to 4-H members who have shown consistent growth in their 4-H program, who have developed their leadership ability, and who have been helpful members in their club and community.
- Members in Grades 11 and 12 who have completed at least 3 years of 4-H and 1 year of junior leadership.

Miscellaneous
- Ambassador (.pdf)
- The 4-H Ambassador application is available for people who are interested in being a 4-H Ambassador. The 4-H Ambassador serves as ambassadors for the county 4-H program, plans special events and activities, and serves as public relations representative.
- Due: Before the Fair
- Federation of Taylor County 4-H Leaders Scholarship (.pdf)
- The Federation of Taylor County 4-H Leaders Scholarship is an available scholarship to any current Taylor County 4-H member who is in the current graduating senior class with at least five years of 4-H club memebership and fulfills all 4-H achievement requirements in the current year.

- Health Statement (.pdf)
- The Health Statement is to be filled out when a youth or adult will be participating in an extended 4-H event. A new form needs to be filled out for each event in order to be up to date on medical conditions.

- MPE (Member’s Program Evaluation)
- Record Book (.pdf)
- The 4-H Record Book is a summary of what you have done for the year. The forms that must be included in each member’s record book are: Younger Member Annual 4-H Summary, Junior 4-H Project Financial Record, or One of the four specialized Financial Forms (required for all projects you are requesting awards in), Photo Sheet (optional), Completed Project Work Books (when available), Examples of Youth Leadership (if applicable). - Secretary Record Book (.pdf)
- Each club needs to complete the Secretary Record Book. This book allows clubs to keep record of what occurred in their club over the past year.

- Space Camp Information/Application Packet (.pdf)
- Each year youth are invited to attend a Space Camp in Huntsville, Alabama in April. Interested participants are asked to read the Space Camp Information/Application Packet.
- Due: January
- Summer Camp Counselor/Chaperone Information (.pdf)
- Each year the 4-H is looking for camp counselors and chaperones for Summer Camp. Anyone interested in applying for one of these positions is asked to please fill out the application and return it to the UW Extension office.
